WHAT IS PLATELET RICH PLASMA?
The treatment with Platelet Rich Plasma is the most innovative in the field of sexual medicine for the treatment of male impotence or erectile dysfunction. It is achieved with the regeneration of the tissues through growth factors contained within the platelets of the patient himself. The P.R.P. How does Platelet Rich Plasma work?
The Plasma, used for the treatment of sexual dysfunctions, is based on a mechanism of action that is the stimulation of the skin cell growth (fibroblasts), the formation and organisation of tpye I collagen, hyaluronic acid, and the skin proteins, achieving structural integrity of the skin, an improvement of the scarring and a stimulation of all structures, like hair follicles and also the cavernous bodies of the penis.

The Proceedings of the National Academy of Sciences and their research team lead by Wayne J.G. Hellstrom, from the Johns Hopkins University in Baltimore (United States) found out that stem cells could help improving the erectile capacity of the penis, reducing the impotence problems men are affected by to a minimum. The study suggests that the stem cells probably contribute to the reconstruction of the erectile tissue, improving the blood flow, the contractile capacity and boosting the production of nitric oxide, which helps keeping the erection.

The growth factors derived from the platelets are biological mediators from the group of proteins that intervene in the process of repair, restoration and regeneration of the cells. They are found in the blood, more specifically in the platelets transported by the plasma. They are generated by the body with the immune characteristics of every individual.

WHAT ARE THE INDICATIONS FOR PLATELET RICH PLASMA?
It is used in surgery as a coagulant, for the scarring of grafts. In aesthetic medicine it’s applied as a repairer of aged tissues; scars, ulcers, alopecia or acne scars, due to the “growth factors” contained in the platelets. The effect can be seen one month after the application. Without any tolerance or rejection problems. What are shock waves?

Shock waves have been used for quite some time in the field of urology, mainly for the treatment of urinary calculi (kidney stones). This same physical principle, but with shock waves of low intensity and high frequency, is the same that is now used for the treatment of impotence. The main mechanism of action of the shock waves is the formation of new blood vessels within the erectile tissue, and the boosting of the relaxation of the vascular endothelium. From 2010 until now, several authors have confirmed the good results and, in the recent years, with sufficient scientific evidence, the different international scientific societies for urology and andrology have endorsed the treatment and included it in their clinical guidelines.

How are the shock waves applied?

The treatment using shock waves is divided into several sessions. There are different systems with slight variations depending on the equipment, but generally the shock waves are applied in sessions of 15-20 minutes, once a week over a time period of 4 weeks. After one month, one more session is performed as a reminder. In each session, the shock waves are applied first on the upper side of the penis and then underneath the scrotum where the root of the cavernous bodies is located. This way, the shock waves cover the entire length of the erectile tissue. The treatment is brief, painless and doesn’t require any kind of analgesia, sedation or anaesthesia. There is also no hospitalisation or observation period needed after each session. The patient arrives at the time of the appointment, the waves are applied, and the patient can return home.

Who can undergo this treatment and who cannot?

The treatment with shock waves is beneficial in cases of erectile dysfunction of vascular origin, in other words, when the arteries of the penis are damaged due to vascular risk factors (tobacco, hypertension, cholesterol, diabetes, obesity, physical inactivity and stress). All of these factors produce arteriosclerosis and the narrowing of the arteries of the whole body, including those of the penis. The treatment is therefore not recommended in cases of erectile dysfunction of neurological origin (spinal injuries, for example), hormonal origin (due to a lack of testosterone), venous leak, or in cases where it is of psychological origin (young males with a lot of anxiety for sexual performance). No improvement can be expected in severe cases that show no reaction at all to the oral intake of medication. This reaction, even though it’s not much, is a good indicator for the presence of viable erectile tissue, potentially improvable. Your doctor will decide whether you are a good candidate for the treatment.

Do the shock waves produce pain or side effects?

No. The shock waves cause kind of a tingling while being applied, but they don’t cause pain and don’t cause any reactions to the penis. There have been no side effects described in medical literature, except for some rare cases of bruising due to blood clotting disorders not previously warned about.
